Central London Community Healthcare NHS Trust
Central London Community Healthcare NHS Trust (CLCH) is a NHS trust which provides community health services to the boroughs of Ealing, Harrow, Barnet, Brent, Hounslow, Richmond, Wandsworth, Merton, Hammersmith and Fulham, Westminster and Kensington and Chelsea, as well as Hertfordshire.[2]

The Trust currently has over 4,500 staff, delivering care to over 2 million people living across London and Hertfordshire. CLCH’s vision is to deliver great care closer to home and its mission is to give children a better start and adults greater independence. [3]The vision is underpinned by four core values to put quality at the heart of everything we do, value our relationships with others, deliver services we are proud of, and make a positive difference in our communities.[4]
The CLCH Strategic Direction (2020 - 2025) sets out four priorities for the Trust to offer leadership in local systems, achieve further integration of services, further develop expertise and achieve a more sustainable, greener future.[5]
Services
CLCH provides more than 90 clinical services, encompassing:
• Adult community nursing including district nursing, community matrons and case management.
• Children and family services including health visiting, school nursing, community nursing, speech and language therapy, blood disorders and occupational therapy.
• End of life care supporting people to make decisions and receive the care they need at the end of their life.
• Long-term condition management supporting people with complex ongoing health needs caused by disability or chronic illness.
• Rehabilitation, neuro-rehabilitation and therapies including physiotherapy, occupational therapy, foot care, and speech and language therapy.
• Specialist services including delivering care for people living with diabetes, heart conditions, Parkinson’s, homeless health services, community dental services, sexual health and contraceptive services.
• Walk-in and urgent care centres providing care for people with minor illnesses and injuries and providing a range of health advice and information.[6]
History
In 2008 a business case was made for integrating the provider arms of three Primary Care Trusts (PCTs) - Hammersmith and Fulham, Kensington and Chelsea, and Westminster.
In July 2008 an alliance was formed with a Joint Provider Committee and single accountable officer, trading under a single name and identity – Central West London Community Services.
In March of 2009 Central West London Community Services was granted single autonomous provider organisation (APO) status by NHS London. In November 2010 the organisation was established as a new NHS Trust, under the new name of Central London Community Healthcare NHS Trust (CLCH).
The organisation merged with Barnet Community Services on 1 April 2011.[7]
Between 2014 and 2015 the organisation expanded to deliver community healthcare services to people living in Brent, Hounslow and Hertfordshire.
CLCH was named by the Health Service Journal as one of the top hundred NHS trusts to work for in 2015.[8]
Between 2015 and 2021 the trust further expanded the boroughs in which it provides services, spanning Harrow, Ealing, Merton, Richmond and Wandsworth.[9]
Recent achievements
In 2020 the Trust launched the first community drive-through testing centre in the country, in response to the Covid-19 pandemic.[10]
In 2021 CLCH set up Hospital at Home in Wandsworth and Merton, one of the first community trust led virtual wards in the country, to provide intensive hospital-level, holistic support for serious conditions, in a patient’s home.[11]
In October 2021 as part of the Trust’s strategic priorities to achieve a more sustainable, greener future, CLCH launched its ‘Green Plan’, a commitment to reducing the Trust’s impact on the environment.[12]
In early 2023 CLCH secured Stonewall’s silver award for leading LGBTQ+ inclusive employers[13] and won the NHS Pastoral Care Quality Award for its international recruitment programme.[14]
In May 2023, CLCH officially opened Furness Ward at the Willesden centre in Brent to support the rehabilitation of patients to enable them to return home sooner.[15]
