Behavioral Risk Factor Surveillance System (BRFSS)
BRFSS is the world’s largest, on-going telephone health survey system.
Cancer Data and Statistics
Statistics for some of the most common kinds of cancer in the United States, based on data from the United States Cancer Statistics: Incidence and Mortality Web-based Report.
Chronic Disease and Health Promotion Open Data
Use these open data to describe the burden of chronic disease and risk factors, identify research gaps, monitor population trends, and evaluate programs.
Chronic Disease Indicators (CDI)
CDI is a cross-cutting set of 124 indicators (with 201 measurements) that were developed by consensus and that allows states and territories to uniformly define, collect, and report chronic disease data.
Chronic Disease State Policy Tracking System
This is a state-based searchable database that provides policy information for preventing and controlling diseases. Currently this database contains nutrition, physical activity, and obesity data.
Chronic Kidney Disease (CKD) Surveillance System
The CKD Surveillance System documents the burden of CKD and its risk factors in the United States population over time and provides the means for monitoring and evaluating efforts to prevent, detect, and manage CKD by both federal and nonfederal agencies.
Data, Trends and Maps
CDC’s Division of Nutrition, Physical Activity and Obesity’s Data, Trends and Maps database is an interactive tool that provides state-specific data about obesity, nutrition, physical activity, and breastfeeding. You can view statistics in a variety of formats, including maps, tables, and trend lines.
Diabetes Data and Statistics
Diabetes Data and Statistics provides resources documenting the public health burden of diabetes and its complications in the United States.
Health-Related Quality of Life (HRQOL)
HRQOL surveillance is used to identify unmet population health needs; recognize trends, disparities, and determinants of health in the population; and guide decision making and program evaluation.
National Assisted Reproductive Technology Surveillance System (NASS)
NASS collects information on Assistive Reproductive Technology (ART) treatment outcomes from all infertility clinics in the United States, and publishes an annual report.
National Health Interview Survey (NHIS)
NHI is a large-scale household interview survey. Survey results are instrumental in providing data to track health status, health care access, and progress toward achieving national health objectives.
National Health and Nutrition Examination Survey (NHANES)
NHANES is designed to assess the health and nutritional status of adults and children in the United States. The survey is unique in that it combines interviews and physical examinations.
National Program of Cancer Registries (NPCR)
NPCR collects data on the occurrence of cancer; the type, extent, and location of the cancer; and the type of initial treatment.
National Youth Tobacco Survey (NYTS)
NYTS is a nationally representative cross-sectional school-based survey of public school students enrolled in grades 6-12.
Pregnancy Risk Assessment Monitoring System
PRAMS collects state-specific, population-based data on maternal attitudes and experiences before, during, and shortly after pregnancy.
Reproductive: Health Data and Statistics
CDC’s Division of Reproductive Health gathers data on maternal and infant mortality, maternal and infant morbidity, adverse behaviors during pregnancy, and long-term consequences of pregnancy.

Smoking and tobacco data and other information from various sources, such as CDC surveillance systems, journal articles, and reports.
Water Fluoridation Statistics
These statistics were prepared using US Census Bureau state population estimates and water system data reported by states to the CDC Water Fluoridation Reporting System.
Youth Risk Behavior Surveillance System (YRBSS)
YRBSS was developed to monitor priority health risk behaviors that contribute markedly to the leading causes of death, disability, and social problems among youth and adults in the United States.
