Letter of Intent
CDC urges you to complete an LOI before you send in your application. The purpose of this letter is to inform CDC that you are interested in applying for funding under PS14-1403 Capacity Building assistance for High-Impact HIV Prevention.
The Letter Of Intent should include the following information:
- Descriptive title of proposed project
- Name, address, telephone number, and email address of the Principal Investigator/Project Director
- Name, address, telephone number, and email address of the primary contact for writing and submitting this application
- Number and title of this funding opportunity
- Funding category for planned program
- For Categories A, B, and C, the three selected program components for planned program
