The national organizations program supports 3 community capacity building and implementation awardees, and 2 dissemination and training awardees. Both types of awardees will work closely to coordinate strategies and technical assistance to maximize their collective impact, increase national and local awareness of community health work, and ensure that targeted communities are able to increase their capacity and achieve sustainable improvements that will continue after program funding ends.
- Community Capacity Building and Implementation awardees will fund two cohorts of up to 20 sub-recipients from among chapters/affiliates to develop or strengthen their coalitions and implement plans that address one or more chronic disease risk factors.
- Dissemination and Training awardees will develop and monitor the uptake and use of tools and trainings to support the capacity building and local community improvements of funded chapters/affiliates and their community coalitions.

Project Overview: AHA will support national organizations in building and strengthening community coalitions to implement population-based strategies that will improve health and reduce chronic disease prevalence. AHA will focus on national leadership and collaboration; acceleration of strategic initiatives; technical assistance, consultation and training; and program management and operations.
The AHA National Center will work with 30-40 communities to strengthen existing coalitions that will develop community action plans that address one or more of the following risk factors for chronic diseases: tobacco use and exposure, poor nutrition, physical inactivity, and lack of access to chronic disease prevention, risk reduction or management opportunities. The action plans will help reduce health disparities, suffering, disability and premature death.

Project Overview: APA plans to improve the capacity of planning and public health professionals; advance community-based strategies providing for equal access to healthcare and nutritious foods; increase opportunities for physical activity; and decrease tobacco exposure and use. Through a collaborative strategy that brings together members of APA and the American Public Health Association (APHA) working in communities, this project will address population health goals by emphasizing health needs in non-traditional public health sectors such as urban and regional planning, transportation, recreation, and real estate development, and others. Furthermore, these efforts will build local capacity of coalitions in at least 30 communities across the United States. While APA and APHA members will serve as the core leaders of these coalitions, other sectors will be encouraged to participate including parks and transportation agencies, social justice organizations, neighborhood associations, faith-based organizations, landscape architecture, and public safety.

Project Overview: ASTDHPPHE will provide training and technical assistance, tailored community assessment reports, and communication about effective community health approaches using a variety of media. ASTDHPPHE also will connect sub-recipients of national organizations funded under Community Capacity Building and Implementation awards with other organizations in their states that can support and sustain their work, and work with awardees to share successes from their community health projects.
ASTDHPPHE will accomplish these goals by increasing collaboration among national, state, and community partners; strengthening awardee capacity; implementing approaches to prevent and manage chronic diseases, including culturally tailored interventions in communities with the greatest burdens of chronic diseases; and disseminating information in communities about the need for community improvements.

Project Overview: NWA will build upon previous Centers for Disease Control and Prevention (CDC) community health programs to promote healthy communities, prevent chronic diseases, and reduce disparities. NWA’s program aims to strengthen social and physical environments to promote positive health by encouraging collaboration across various sectors, implementing evidence- and practice-based strategies, and measuring the effects of prevention activities.
NWA will support 36 local WIC agencies in communities across the country. Each agency will create or enhance a local coalition, perform a community needs assessment, and develop and implement evidence- and practice-based strategies addressing lack of access to chronic disease prevention, risk reduction, and management opportunities, and poor nutrition. Local coalition leadership teams will partner with a WIC agency staff member, an obstetrician/gynecologist, and a WIC mother or patient supporter. NWA will also collaborate with the American College of Obstetricians and Gynecologists, other funded national organizations, and CDC on communication and dissemination activities.

Project Overview: SOPHE will engage its national and chapter members, other national partners, and a number of health experts to support a national framework to prevent chronic diseases and promote health. These efforts are needed to assist funded partners in the development of community health strategies. SOPHE will enhance cross-sector collaboration to promote and improve opportunities for healthy eating and physical activity, and to provide tobacco-free environments in communities.
SOPHE recognizes the need to develop informational materials that are easily understood by non-public health professionals. SOPHE will provide support for capacity building and technical assistance in the following areas: workforce training, leadership, partnerships, social marketing/health communications, resource development, and organizational development.
