Cooperative Agreements

CDC supports state health departments through two cooperative agreement programs.

Under the State Oral Disease Prevention Program (DP1307), CDC funds 21 state health departments. These awards range from $230,000 to $310,000 per year, renewable through fiscal year 2017.

The purpose of DP1307 is to help state health departments build or maintain public health capacity through collective impact—the commitment of a group of people from different sectors to a common agenda for solving a specific societal problem. States with public health capacity also use funds to implement proven community and clinical interventions that target vulnerable and underserved populations.

Under Models of Collaboration for State Chronic Disease and Oral Health Programs (DP1609), CDC provides support to 6 state health departments to strengthen intra-department collaboration between their chronic disease and oral health programs. Each state is receiving $250,000 per year for two years to implement pilot projects to improve the oral health of state residents while preventing and controlling chronic diseases, such as high blood pressure (hypertension) and diabetes and related risk factors such as tobacco use and obesity.

The map and table below indicate the 21 states that have been awarded funding under DP1307 and the six states funded under DP1609. Both funding opportunities run through August 2018.
