Systematic Review of Programs Treating High-Need and High-Cost People With Multiple Chronic Diseases or Disabilities in the United States, 2008–2014

A flow chart describes the selection of the 27 articles in the study that met all criteria. A total of 1,726 articles were identified by the original search. Twenty-six additional studies were identified by hand-searching for a total of 1,762 articles screened. We excluded 1,617, leaving 145 articles that met all criteria on the basis of title and abstract screening. Of these, 89 had an ineligible target population, 22 lacked an intervention, and 7 reported no outcomes. A total of 27 articles met all our criteria.
