Supporting Documentation

You will need to provide copies of documents that show you were present and/or worked at the World Trade Center site(s). If you can't get any of them, you will need to write a short report explaining:

  1. What you did to try and get copies of those records
  2. Why you can’t include them with your application.

FDNY Responders

Please provide copies of documents that show all of these things:

  1. What your work or volunteer activity was at the former World Trade Center site(s),
  2. Your location at the former WTC site(s), and
  3. The time period that you were working or volunteering at the site(s).

Examples of acceptable supporting documentation include copies of:

    • A letter from FDNY stating that you participated in WTC-related activities on or after September 11, 2011
    • An award or letter given by FDNY that commends your WTC activities
    • Timesheets or overtime reports from FDNY that show the day(s), hours worked, and location(s) of World Trade Center activities

If you cannot get copies of these supporting documents, please write a short report and send it in with your form. In the report, explain:


  • How you tried to find documentation of the work you did
  • Why you cannot include documentation with your application

Responders


Please provide copies of documents that show all of these things:

    • A letter from the Workers’ Compensation Board that reports a paid and/or active worker’s compensation claim for your World Trade Center activities
    • Timesheets or overtime reports that show the day(s), hours worked, and location(s) of World Trade Center activities

If you cannot get copies of these supporting documents,
please write a short report and send it in with your form. In the report, explain:

  • How you tried to find documentation of what you did
  • Why you cannot include documentation with your application

Survivors

Please provide copies of documents that show both of these things:

  1. The location of your home, work, or school, child care, or adult day care in the New York City disaster area, and
  2. The time period that you lived, worked, or attended school at that location.

Examples of acceptable supporting documentation include copies of:

  • A lease or utility bill with your name showing your residence between September 11, 2001 and July 31, 2002
  • A pay stub if it shows your work location between September 11, 2001 and July 31, 2002
  • A letter from your employer, day care center, school, or volunteer organization that states your location between September 11, 2001 and July 31, 2002
  • A student identification card with an associated date between September 11, 2001 and July 31, 2002
  • A school transcript from the Fall 2001 semester

If your home, work, or school is not in the New York City Disaster Area, but on September 11, 2001, you were present in the New York City Disaster Area, your supporting documentation could include copies of:

  • Receipts showing location and the date of September 11, 2001;
  • Written statements from people who were with you , or
  • A detailed description of your activities in the New York City Disaster Area on September 11, 2001.

If you cannot get supporting documentation, please write a short report and send it in with your form. In the report, explain:


  • How you tried to find documentation of your presence in the area
  • Why you cannot include documentation with your application

Shanksville, PA/Pentagon Responders


Please provide copies of documents that show all of these things:

  1. Your rescue, recovery, demolition, debris cleanup, or related services activities;
  2. The date(s) that you worked or volunteered onsite; and
  3. Your location(s) during the relevant work/volunteer activity.

Examples of documents/records include, but are not limited to, the following:

  • Written statement by an employer or organization that confirms your relevant work/volunteer activity and your work/volunteer location during the days you stated above;
  • Work ID, site ID, rosters, memo books, pay stub, 1099, W-2, W-4, tax return, or other documents that show your name, your relevant work activity, and your relevant work location during the days you stated above;
  • Site ID, rosters, award certificates, or other documents that show your name, your relevant volunteer activity, and your relevant volunteer location during the days you stated above; or
  • Your employer's (or any other work-related) website, news articles, or other information that shows your name, your relevant work/volunteer activity, and your relevant work/volunteer location during the days you stated above.

If you cannot get copies of these supporting documents,
please write a short report and send it in with your form. In the report, explain:

  • How you tried to find documentation of what you did
  • Why you cannot include documentation with your application
