Holotype: URM-P 5980. [Paratypes: ASIZP 59817 (1); BMNH 1998.11.9.1 (1); BPBM 22329 (1), 35137 (1); URM-P 335 (1), 18508 (1); USNM 350960 (2)].
Type locality: Off east side of Sesoki Island, Okinawa, Ryukyu Islands, reef in 4 meters.
