Type locality: Haraichi Formation, Annaka Group; "outcrop along the Kabura River, Yoshii, Takasaki, Gunma Prefecture, Japan".
Holotype: GMNH -PV-2401
Eponymy: Masaru Shimizu, "who found the type specimen and donated it to GMNH" [Kimura & Hasegawa, 2010: 578]
