Holotype: BPBM 9298. [Paratypes: AMS I.21589-001 (1); ANSP 144103 (1); BMNH 1980.7.23.2 (1); BPBM 6934 (2), 9100 (1), 9093 (1), 13506 (1), 13674 (1), 20691 (3); CAS 46535 (1); MNHN 1980-1343 (1); USNM 208290 (1)].

Type locality: Outside barrier reef, 300 meters east of pass to Papetoai Bay, Moorea, Society Islands, depth 60 feet.
