State and Territory Initiative

This program is no longer funded. Learn more about current DCH programs.

Funds in the amount of $120 million were provided to support states and territories in promoting wellness and preventing chronic disease through statewide policy and environmental change for chronic disease prevention and to increase tobacco cessation through expanded quitlines and tobacco cessation media.

The three primary components of the State and Territories Initiative include:

  1. Statewide Activities: Activities will support and sustain healthy behaviors related to obesity control, nutrition, physical activity, and tobacco control and prevention.
  2. Special High Impact Initiative: Implementation of at least one additional high–impact strategy to achieve health equity and eliminate health disparities in the area of physical activity, nutrition, tobacco, or a combination of these.
  3. Tobacco Cessation through Quitlines and Media: Funding will expand tobacco quitlines, in concert with expanding smoking cessation media campaigns. States and territories receive funding based on the number of smokers in their jurisdiction. Additional funds will be used for national efforts to support surge capacity, additional quitline monitoring, and quality improvement measures.
