Denominators for computing 2000–2011 case rates were obtained from the Annual Estimates of the Resident Population by Sex, Race, and Hispanic Origin for the United States: April 1, 2000, to July 1, 2009 (accessed July 20, 2012) and Annual Estimates of the Resident Population by Sex, Race, and Hispanic Origin for the United States: April 1, 2010 to July 1, 2011 (accessed July 20, 2012).

Case counts for race categories (American Indian or Alaska Native, Asian, Black or African American, Native Hawaiian or Other Pacific Islander, and White) do not include persons of Hispanic ethnicity or multiple race.

Data for all years updated through June 10, 2013.
