It's not possible to get a Sim pregnant on the original Sims 1, but that doesn't mean your Sims can't have kids! There are two ways to make your Sim have a baby in The Sims; one only needs the base game, and the other requires the Livin' Large expansion pack.

Method 1

Through Kissing

    Choose a heterosexual couple. Create a family with an adult male and adult female Sim, or load an existing family with an adult male and adult female Sim.

    Fill your Sims' needs. If your Sims are in a bad mood, you can't direct them to have a baby. Make sure most of their motives are relatively full.

    Build up your Sims' relationship. Sims can only have a baby together if they're in a romantic relationship. If your Sims aren't already in a relationship, have them build up their relationship using various actions under Talk, Hug, Entertain, Flirt, and so forth.
    • If your Sims are in a relationship, they'll have a heart under each others' portraits in the Relationships panel.

    Direct your Sims to kiss repeatedly. Click on your Sim's partner and choose Kiss; if you have Hot Date installed, choose a random type of kiss. Continue directing them to kiss for as long as necessary.
    • Make sure their motives are still high, or they won't want to have a baby (and may start rejecting interactions).
    • If you have Hot Date, pick different kisses each time; don't only pick one type of kiss. Your Sims will get annoyed and start pushing their partner away.

    Respond to the "Should we have a baby?" popup. After your Sims have kissed multiple times, a popup might appear, with one of the Sims asking "Should we have a baby?" Click Yes.
    • The popup won't appear every time your Sims finish kissing. You'll probably need to try a few times before it works.

    Name the baby. A bassinet will appear nearby, and a dialogue box will pop up telling you whether the baby is a boy or a girl. Enter in the baby's name, hit OK, and begin caring for your Sims' new baby!

Using the Heart-Shaped Bed (Livin' Large)

    Build up your Sims' relationship. Sims need to be in a romantic relationship to have a baby together. If your Sims aren't already in a relationship, have them build up their relationship using various actions under Talk, Hug, Entertain, Flirt, and so forth.
    • Sims in a relationship will have a heart under their partner's portrait in the Relationships panel.

    Fill your Sims' needs. Sims will refuse to have a baby if they're in a bad mood, and the "Play in Bed" interaction will cause some of your Sims' motives to drop a little bit. Make sure most of their motives are relatively full.

    Purchase the Vibromatic Heart Bed. It's located under Comfort > Beds, and costs 4,500 Simoleons. Place it in a location where both sides can be accessed.

    Direct one Sim to turn on the bed. Pick one of the Sims, click on the bed, and choose Vibrate. Your Sim will pay 20 Simoleons to activate the bed, and then remove their clothes and get in bed.

    Direct their partner to Play in Bed. Select your Sim's partner, click on the bed, and choose Play in Bed. Your Sim's partner will remove their clothes, get into bed, and the two Sims will disappear under the covers together.

    Say yes to the "Should we have a baby?" popup. Once your Sims have finished Playing in Bed, you might get a popup from one of the Sims, asking "Should we have a baby?" Choose Yes.
    • You might have to try a few times before the popup appears; it doesn't happen every single time.

    Name the baby. Once the popup disappears, a bassinet will appear near your Sims, and you'll get another popup box telling you whether the baby is a boy or a girl. Enter in the baby's name, hit OK, and begin caring for your Sims' new baby!

Warnings

  • Make sure your Sims are a safe distance from the edges of the lot. If they're too close to the edges, the baby could spawn outside the lot boundaries.
